Home health care helps people of all ages remain in the comfort of their own home. From the pediatric patient to the frail elderly adult and all those in between, services are provided which are suitable to the patient’s needs, as prescribed by their physician. Home care allows the patient to recuperate from an illness, receive long term care for the chronic patient, assist the patient with activites of daily living and so much more.
Our professional nurse will do a comprehensive assessment, consult with your physician, and come up with an individually designed plan of care. The professional services are delivered by our very caring and competent nurses, and our physical, occupational and speech therapists. A medical social worker can also be involved when needed.
Paraprofessional services are delivered by home care aides. They have been trained to help with activities of daily living like bathing and dressing, and the chores you need done like cooking and light housekeeping. They can also assist with mobility and help the patient maintain independence. In addition, they provide companionship which is so important emotionally to the person in their home.
The plan of care can be limited to a few hours a day, or short visits by the appropriate discipline or a longer shift. In certain cases, live in help is appropriate to maintain the patient in their home. Our nurses are available 24 hours a day to answer your questions and take care of any questions which may arise.
